Nalcrom Survey Report

In February and March 2021, MSC conducted a survey on access to the medication Nalcrom. This survey was in part in response to concerns raised by patients over appearance changes and efficacy changes in recent batches of Nalcrom. Following the completion of the survey, we have created a report that summarises the data collected and our efforts to advocate on behalf of our patient population.

In addition to publicly sharing this report on our website, we have shared it with Sanofi Canada and Health Canada.

• Focus pédiatrique: maintenir un focus à travers les objectifs de la SCM afin d’offrir des ressources utiles aux familles et aux jeunes qui vivent avec un diagnostic de maladie des mastocytes.

La SCM recherche un bénévole impliqué qui sera passionné pour favoriser la sensibilisation face aux maladies des
mastocytes et l’amélioration de la qualité de vie des patients à travers le Canada. Nous travaillons en collaboration afin d’établir une stratégie et une direction pour l’organisation mais aussi en s’engageant à implanter des initiatives pour nous amener encore plus près de nos objectifs stratégiques.

Engagement - Le membre du conseil d’administration peut s’attendre à s’engager bénévolement environ 10 heures par mois. Le poste de directeur implique la participation à une réunion du conseil d’administration par mois et une réunion de comité par mois (2 heures chacune). Le conseil d’administration peut tenir des rencontres de travail supplémentaires afin de soutenir la collaboration et la mise en place de solutions à différentes problématiques.

Terme d’engagement – 3 ans

Expérience – Idéalement, les bénévoles au sein du conseil d’administration ont une expérience dans l’un des domaines suivants:

• Sensibilisation et éducation;
• Support;
• Représentation et collaboration avec les services de santé;
• Professionnel de la santé avec un intérêt marqué pour les maladies des mastocytes;
• Collecte de fonds;
• Communication et médias sociaux;
• Administration générale et comptabilité.

Qualifications – Aucun expérience antérieure au sein d’un conseil d’administration n’est nécessaire mais elle sera considérée comme un atout.

En somme, la SCM recherche des personnes dynamiques, qui souhaitent faire une différence dans la communauté. Mastocytosis Society Canada (MSC) is a registered Canadian Charity committed to assisting patients, caregivers and health care professionals learn more about mast cell disorders and lean on each other for support. We are here to support patients by:

• Awareness and Education: Raise awareness and provide education on mast cell disease to patients, caregivers, healthcare professionals and the public.
• Guidance and Support: Provide support to patients who are seeking diagnosis or have been diagnosed with a mast cell disease.

Available Position: Board of Directors

MSC is looking for volunteer to take on who are passionate about raising awareness of mast cell diseases and
improving the quality of life for patients across Canada. We work together to set strategy and direction for the
organization, but also roll-up our sleeves to implement initiatives that bring us closer to meeting our strategic goals.

Commitment – Board Members can expect to volunteer approximately 10 hours per month. A director will be expected to attend 1 board meeting and 1 committee meeting a month (2 hours/each). The Board of Directors may hold additional periodic working sessions meant for collaboration and problem solving.

Length of Term – 3 years

Experience – Ideally, board volunteers will have experience in one or more of the following areas:

• Awareness and Education
• Guidance and Support;
• Representation and Health Care Partnerships
• Healthcare practitioner with a keen interest in mast cell disease;
• Fundraising;
• On-line communication and social media;
• General accounting or governance skills.

Qualifications – No previous board experience is required; however, it would be considered an asset.

Overall, MSC is looking for dynamic individuals who want to make a difference in their community. The volunteer board position is an excellent opportunity to leverage your previous experience, add value, make a contribution and give back to the community while gaining experience in a Board position.

Nous souhaitons vous offrir une mise à jour concernant le médicament nommé Nalcrom et nos récentes discussions avec Sanofi Canada.  Le 4 mars 2021, nous avons partagé avec vous une mise à jour concernant la rupture des stocks de Nalcrom par Sanofi Canada.  La déclaration de rupture des stocks par Sanofi Canada faisait suite aux préoccupations exprimées par des patients relativement à la qualité et l’efficacité des lots récents de Nalcrom distribués aux patients canadiens.  Cette rupture des stocks déclarée par Sanofi Canada, disponible sur le site web Drug Shortage Canada, mentionnait que la date estimée de la fin de cette rupture serait le 23 avril 2021.  

Le 23 avril 2021, Sanofi Canada a révisé cette déclaration sur Drug Shortages Canada et repoussé la date de fin de la rupture des stocks pour le 21 mai 2021.  Nous sommes préoccupés par cette projection et avons communiqué avec Sanofi Canada ainsi que Santé Canada pour faire le suivi et mentionner nos préoccupations pour la population de patients que nous représentons.

Alors que les patients n’ont pas accès au médicament Nalcrom pendant cette rupture des stocks déclarée, quelques patients nous ont souligné l’option d’avoir accès à la médication avec l’aide de pharmacies magistrales. De cette façon, les patients ont pu travailler avec l’aide du professionnel prescripteur et une pharmacie magistrale pour avoir accès au composant cromoglycate de sodium (le composant actif du Nalcrom).  Pour toute information concernant cette médication magistrale, nous recommandons que vous puissiez discuter avec votre prescripteur et la pharmacie magistrale la plus proche.  Voici ici le lien pour Association of Compounding Pharmacist of Canada, incluant une carte indiquant les différentes pharmacies magistrales au Canada.

Considérant les coûts, nous reconnaissons que le prix du médicament Nalcrom est un fardeau financier pour plusieurs patients et nous sommes conscients que différentes assurances (privées ou publiques) ne couvrent pas les coûts de la médication magistrale. Certains patients ont partagé avec nous la possibilité que leur plan d’assurances ait pu couvrir les frais de la médication magistrale en démontrant le fait que la rupture des stocks de Nalcrom est en vigueur. Cette preuve peut être la notification officielle retrouvée sur le Drug Shortages Canada ou autres liens offerts sur notre site web.  Bien que nous reconnaissons que ce ne sont pas tous les patients qui pourront obtenir la couverture de la médication magistrale par leur plan d’assurances, nous partageons cette information pour ceux qui estiment cela aidant.

Nous allons poursuivre nos représentations concernant la rupture des stocks et les préoccupations liées à la qualité du médicament Nalcrom.  Lorsque la rupture des stocks sera résolue, nous planifions poursuivre nos représentations pour un meilleur accès au Nalcrom et une couverture financière améliorée de ce médicament, tant auprès des compagnies privées ou des systèmes publics.

Chers membres de notre communauté,

Nous souhaitons vous offrir des informations à propos du médicament Nalcrom et de nos
discussions récentes avec Sanofi Canada. En décembre 2020, la Société Canadienne de la
Mastocytose (SCM) a été informée par des membres de notre communauté qu’il y avait
certaines préoccupations en regard de la qualité et de l’efficacité de Nalcrom concernant des
lots spécifiques et récents. Ces préoccupations ont été soulevées avec une autre
problématique d’approvisionnement éprouvée au fil des années par certains patients.

Nous avons donc mis en place un sondage pour obtenir des informations concernant ces
préoccupations d’approvisionnement et de qualité du produit qui nous avaient été
communiquées. Ce sondage a débuté le 12 février 2021 et est encore en vigueur jusqu’au 15
mars 2021.

Parallèlement à ce sondage qui continue d’accumuler les réponses jusqu’au 15 mars 2021,
nous avons immédiatement mis en place des actions pour évoquer ces préoccupations.
Depuis le 2 février 2021, nous avons participé à des réunions hebdomadaires avec un
représentant de Sanofi Canada. En représentant nos membres, nous avons partagé nos
résultats préliminaires du sondage (48 répondants) et avons évoqué la nécessité de
considérer et résoudre rapidement les plaintes de nos membres concernant la qualité et
l’efficacité du médicament Nalcrom. Le 26 février 2021, nous avons acheminé une lettre à
Santé Canada pour évoquer la situation, partager avec eux les résultats préliminaires de notre
sondage et demandé leur support pour résoudre les préoccupations soulevées par la
population touchées concernant les récents lots de Nalcrom. Notre lettre a été reçue par la
Direction Générale des opérations réglementaires et de l’application de la loi de Santé Canada
ainsi que par la Direction de la conformité des produits de santé. Notre lettre a également été
acheminée au Programme de Vigilance Canada. Nous allons vous transmettre toute
information supplémentaire ultérieurement.

En date du 3 mars 2021, nous avons eu un échange téléphonique de suivi avec le
représentant de Sanofi Canada qui nous informait des éléments suivants. En regard des
événements rapportés par des patients et par les informations partagées par la SCM, Sanofi
Canada allait pousser l’enquête pour examiner le prochain lot de Nalcrom qui sera distribué au
Canada. Pour s’assurer de pouvoir faire ce travail, Sanofi Canada a déclaré en date du 2 mars
2021 qu’il y aura un arrêt temporaire d’approvisionnement de Nalcrom au Canada. Sanofi
Canada estime que le produit serait de nouveau disponible le 23 avril 2021. Vous trouverez
en document joint la lettre de Sanofi Canada qui évoque ces informations à ce sujet et qui est
acheminée à tous les professionnels de la santé concernés.

Nous comprenons que ces informations apporteront un lot de déception pour plusieurs
patients, mais nous souhaitions partager ces informations le plus rapidement possible pour
que vous puissiez prendre des décisions en regard de ces informations. Nous allons
également partager toute information supplémentaire lorsqu’elle sera disponible.

Nous continuons à faire valoir les intérêts de la population que nous représentons et nous
nous assurons que Sanofi Canada ainsi que Santé Canada soient informés qu’il s’agit d’un
médicament qui est offert à une population de personnes touchées par une maladie rare et
qu’aucune autre source générique de ce produit n’est disponible. Nous encourageons les
personnes ayant des préoccupations en lien avec la qualité et/ou avec des réactions
indésirables aux récents lots de Nalcrom à informer Sanofi Canada et Santé Canada
directement. Vous pouvez décrire votre situation particulière en complétant une plainte en
contactant Sanofi Canada at 1-800-265-7927. En ce qui concerne Santé Canada, vous
pouvez soumettre un Formulaire de déclaration d’effets secondaires avec ce lien.

Pour connaître l’évolution de la situation quant à l’enquête sur la qualité du produit et sur la
rupture de stock annoncée, nous avons prévu de travailler en collaboration avec Sanofi
Canada tout en souhaitant adresser la nécessité pour les patients d’avoir accès au Nalcrom de
façon continue et sécuritaire, puisque le problème d’approvisionnement réapparaît à
l’occasion. Nous travaillons activement pour s’assurer que les patients vivant avec une
maladie des mastocytes puissent avoir accès de nouveau et le plus rapidement possible au
Nalcrom et nous avons mentionné à quel point cette situation amène un risque à la population
vivant avec ces maladies rares.

Si la situation actuelle de rupture de stock vous affecte ou affecte un proche, nous
recommandons que les patients s’adressent à leur professionnel de la santé pour discuter des
étapes à mettre en place dans ce contexte.

Beginning in December of 2020, the Mastocytosis Society of Canada (MSC) began receiving reports from patients of concerns regarding Nalcrom. The medication Nalcrom (also known as Cromolyn Sodium Oral) is recognised as a first line therapy in the management of the symptoms of mast cell disorders which include abdominal pain, diarrhea, nausea, bone pain, cognitive dysfunctions, and anaphylaxis. Nalcrom is recognized as a "mainstay" or "key" medication for patients with mastocytosis and other related mast cell disorders. Canadian patients rely exclusively on Sanofi Canada to supply Nalcrom as there is no other approved version of this medication in Canada.

Following these reports, MSC conducted a patient survey from February 12, 2021, to March 15, 2021. Of 55 returned surveys, thirty-nine (39) reported physical changes to the medication (significantly less full capsules and dissolving challenges). Upon follow up, 22 reports of adverse events were coincident with the commencement of batches with identified appearance changes. Adverse events included reports of anaphylaxis (4, including 3 reports of multiple instances of anaphylaxis), gastrointestinal symptoms (18), allergic-like symptoms (14, including hives, flushing, itching) and one (1) pediatric case. Eight (8) respondents indicated that their adverse reactions were eliminated by returning to older Nalcrom batches or compounded cromolyn sodium. Post-Shortage Update on Quality Concerns with Nalcrom

Sanofi Canada declared a shortage on March 3, 2021, that was extended on April 23, 2021 to May 21, 2021. The shortage was declared resolved May 21 2021 on the Health Canada drug shortages website. During this almost 3-month period, patients reliant on Nalcrom for symptom management of their rare disease had no access to any alternative option(s) to replace this medication.

After the shortage was declared resolved by Sanofi on May 21, 2021, patients continued having difficulty accessing Nalcrom from their pharmacy well into July, 2021. Of the few patients who have been able to access the new batch of Nalcrom released after the shortage (Lot 00078N and lot 00104N; exp. Mr/24), 8 have reported continued issues with the quality of the Nalcrom capsules, citing nausea, diarrhea, abdominal pain, anaphylaxis and an overall increase in symptoms.

We have been informed by at least 4 patients that their adverse events were reported to Sanofi Canada since June, 2021 using the new batches of Nalcrom (00078N and 00104N). While this may appear to be a reduction in reports of adverse events since the release of the June 2021 Nalcrom batches, we would like to highlight that many patients have informed us they have either have not been able to access the latest Nalcrom batch from their pharmacy or have entirely switched to compounded sodium cromoglycate while quality issues remain unresolved.

All MSC survey respondents have been encouraged to report adverse events directly to Sanofi Canada and Health Canada. An examination of the Health Canada Adverse Event Database also points to a spike in adverse event reporting on the drug Nalcrom since December 2020. There have been a total of 41 AE reports on Nalcrom since 1982. Between 1982 and 2019, there were a total of 23 for a rate of 0.62 adverse event reports per year. Between December 18, 2020 and March 31 2021, there have been 18 adverse event reports. This is equivalent to a rate of 54 adverse events per year. This represents an 87-fold increase in adverse event reporting. The database does not show any reports beyond March 31, 2021, however based on our communications with our rare disease patient population we expect there to be more. We have reached out personally to every patient that has reported adverse events to MSC to confirm that they have also reported this to Health Canada and Sanofi Canada. We are continuing to follow up with patients to encourage further adverse event reporting.

Quality concerns with the drug Nalcrom is an ongoing issue within our patient population, both with old batches from Fall/Winter 2020 and new batches released in June 2021. It has been 9 months since our patient population first identified quality issues with the Nalcrom batches. Thus far, the investigations by Sanofi Canada and Health Canada remain ongoing and there has been no resolution. Our patients have no alternative medications and accessing the compounded sodium cromoglycate is not a sustainable solution, as it is not covered by public and private insurance plans and has been reported as less effective than the marketed Nalcrom (prior to the recent quality issues). There has been no communication from Sanofi Canada or Health Canada since July 2, 2021.

Dear Members of our Community, 

We are writing to provide an update regarding the medication called Nalcrom and our recent discussions with Sanofi Canada. On March 4, 2021, we shared an update with our patient population regarding the declared shortage of Nalcrom by Sanofi Canada. This shortage declaration by Sanofi Canada was made in response to significant concerns raised by patients with the quality and efficacy of the recent batches of Nalcrom distributed to patients in Canada. The declared shortage by Sanofi Canada, available on the Drug Shortages Canada website, stated that the estimated end date would be on April 23, 2021. 

On April 23, 2021, Sanofi Canada revised this submission to Drug Shortages Canada and extended the estimated end-date of the shortage to May 21, 2021. We are concerned about this extension and have reached out to both Sanofi Canada and Health Canada to follow up on this and advocate on behalf of our patient population.

If patients are unable to access the medication Nalcrom during this declared shortage, some patients have reported to us the option of accessing the medication through a compounding pharmacy. In this instance, patients have been able to work in partnership with their prescribing healthcare practitioners and a compounding pharmacy to access the drug sodium cromoglicate (the active drug within Nalcrom medication). For any further inquiries about compounding medication we recommend you reach out to your prescribing physician and the nearest compounding pharmacy to you. Included here is a link to the Association of Compounding Pharmacist of Canada, which includes a map with many available compounding pharmacies across Canada. 

In regards to cost, we recognise that the cost of the medication Nalcrom is a financial burden for many patients and recognise that many insurance and public pharmacare plans do not cover compounded medications. Some patients have shared with us that they have been able to have their insurance companies cover the costs of the compounded version of the medication upon provision of proof of the current shortage. Examples of documentation include the official notice on Drug Shortages Canada and links to our website updates. Although we recognize that  not all patients will be able to have the compounded medication financially covered by an insurance company, we share this information for those that it might be relevant to.

We will continue to advocate for the timely resolution of both the shortage and the quality complaints regarding the Nalcrom medication. Following the resolution of the shortage, we plan to take steps to advocate for greater stability of access to the Nalcrom drug and better financial coverage of the medication in private and public drug plans. We will provide updates as they become available.

Dear Members of our Community, 

We are writing to provide an update regarding the medication called Nalcrom and our recent discussions with Sanofi Canada. In December 2020, we were informed by members of our patient community of concerns with the quality and efficacy of the Nalcrom medication from the most recent batches. Concerns regarding the quality of the medication were raised in conjunction with concerns regarding the supply of the medication, with many patients expressing to us difficulties accessing the medication from their pharmacy. 

We took action by launching a survey to collect information on both the supply challenges and the quality concerns that had been raised to us. This survey launched February 12, 2021 and will remain open until March 15, 2021. 

Despite the survey continuing to collect responses until March 15, 2021, we took immediate steps to address this issue. Since February 2, 2021 we have participated in weekly meetings with a representative from Sanofi Canada. We shared with them the preliminary results of our survey (now 48 respondents) and advocated on behalf of our patient population  the need to investigate and promptly resolve the quality and efficacy complaints that had been raised by patients. On February 26, 2021, we sent a letter to Health Canada to make them aware of the current situation, share with them our preliminary survey results, and request their support in resolving concerns raised by our patient population related to the most recent batches of Nalcrom. Our letter has been received by the Health Canada Regulatory Operations and Enforcement Branch (ROEB) and Health Product Compliance Directorate (HPCD). Our letter has also been forwarded to the Canada Vigilance Program.  We will share any follow-up information.

Yesterday, March 3, 2021, we had a follow up call with a Sanofi Canada representative and were informed that based on adverse events reported by individual patients and the information we have shared with them, that they are taking additional steps to investigate and examine the next batch of Nalcrom medication that will be distributed in Canada. To ensure that they are able to conduct the reviews of the next batch that they deem necessary, Sanofi Canada on March 2, 2021 declared a temporary shortage of the Nalcrom medication. The company anticipates that the medication will once again become available to patients in Canada on April 23, 2021. We have attached to this statement a copy of the letter that Sanofi Canada has provided to all of your healthcare professionals on the matter.

We realize that this update will come as a disappointment to many of those in our patient population, however we  wanted to share this new information as soon as possible to allow you to make decisions based on all current information. We will share additional updates as they become available.

We will continue to advocate on behalf of our patient population and have ensured that both Sanofi Canada and Health Canada know that this is a medication for a rare disease population that has no generic source of accessing this medication. We encourage individuals that have concerns regarding the quality of and/or experience adverse events to the most recent batches of Nalcrom medication to inform Sanofi Canada and Health Canada directly.  You can raise concerns over a medication and file a complaint by calling Sanofi Canada at 1-800-265-7927. Following the resolution of the current quality investigation and declared shortage we have arranged with Sanofi Canada to work collaboratively towards addressing the longstanding supply issues that patients have experienced with Nalcrom. We want to reiterate, however, that at the moment our key priority is resolving the Nalcrom medication quality concerns and the newly declared shortage. We are actively working towards ensuring that individuals living with mast cell diseases have access to Nalcrom medication as soon as possible and have made clear the current situation poses risks to this rare disease population. 

If the newly declared shortage will affect you or someone you are caring for we recommend that patients reach out to their prescribing healthcare professional to discuss the appropriate next steps.

McGill University Health Centre researchers seek participants for a study on Mastocytosis

MASTER: Mastocytosis Assessment and Treatment Evaluation Registry

We are now initiating a new study that will look at the factors associated with the history and management of mastocytosis including mastocytosis flare ups, and resolution in children and adults. This information will help the research team learn more about the diagnosis, impact and management of mastocytosis, will help bridge knowledge gaps and hence contribute to better management of patients with mastocytosis.

Our research team, led by Dr. Moshe Ben-Shoshan, from the Division of Allergy and Clinical Immunology at the Montreal Children’s Hospital, is interested in learning more about the clinical characteristics and management of mastocytosis and how it affects quality of life.

To be eligible, individuals should be diagnosed by a physician as having mastocytosis.

If you agree to participate, you will be asked to complete an initial short questionnaire about your, or your child’s, experience with mastocytosis, medical history, and management of flare ups. These will take no more than 30 minutes to complete. You will also be asked to answer a short telephone interview annually related to your, or you child’s, mastocytosis.

Étude MASTER (Mastocytosis ASsessment and Treatment Evaluation Registry)

Nous amorçons une étude qui traitera des facteurs associés aux antécédents et à la gestion thérapeutique de la mastocytose, notamment les crises de mastocytose ainsi que sa résolution chez l’enfant et l’adulte. Ces renseignements permettront à l’équipe de recherche de mieux en connaître le diagnostic, l’impact et la prise en charge et de combler les lacunes du savoir afin d’améliorer la gestion thérapeutique des patients aux prises avec la mastocytose.

Notre équipe de recherche, dirigée par le docteur Moshe Ben-Shoshan du service d’allergie et d’immunologie clinique de l’Hôpital de Montréal pour enfants, souhaite en apprendre davantage sur les caractéristiques cliniques et la prise en charge de la mastocytose ainsi que sur son impact sur la qualité de vie.

Pour être admissibles, les candidats doivent avoir reçu un diagnostic de mastocytose de la part d’un médecin.

Si vous acceptez de participer, vous devrez répondre à un bref questionnaire de départ portant sur votre vécu (le vécu de votre enfant) avec la mastocytose, vos (ses) antécédents médicaux ainsi que la gestion des crises. Cela ne devrait pas prendre plus de 30 minutes. Nous vous demanderons également de répondre à une courte entrevue téléphonique annuelle portant sur votre mastocytose ou sur celle de votre enfant.
